During Tega Cay Tennis LLC camp sessions, photographs and video recordings may be taken of participants for use in promotional materials, including but not limited to social media platforms (Facebook, Instagram, etc.), the camp website, flyers, and other marketing materials. These images may be used without compensation.

By checking the box below, you grant Tega Cay Tennis LLC and Coach Keith Womble permission to photograph and/or record your child and to use those images/videos for promotional purposes. You waive any right to inspect or approve the finished content.

Assumption of Risk: I understand and acknowledge that participation in tennis and physical activities involves inherent risks, including but not limited to: sprains, strains, fractures, heat-related illness, and other injuries. I voluntarily assume all risks associated with my child's participation in Tega Cay Tennis LLC camp activities.

Release of Liability: In consideration of my child's participation, I hereby release, waive, and discharge Tega Cay Tennis LLC, Coach Keith Womble, and all associated staff, volunteers, and facility owners from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury — including death — that may be sustained by my child during participation in camp activities, whether caused by negligence or otherwise.

Medical Authorization: In the event of an injury or medical emergency, I authorize Tega Cay Tennis LLC staff to seek emergency medical treatment for my child if I cannot be reached. I agree to be responsible for all medical costs incurred.

Indemnification: I agree to indemnify and hold harmless Tega Cay Tennis LLC and Coach Keith Womble from any loss, liability, damage, or cost they may incur arising from my child's participation.
